Wandsworth - Lower Back Pain app
This NHS supported app is now available to help people in Wandsworth. It enables you to self refer into physiotherapy should you need to. Please click on the Image below to get your access code. The app is designed work with you, alongside your GP
practice, to:

Patient Access
Online Patient Access enables you to set up a personal account hence creating a secure and dedicated login for you to access your records on our clinical system. Using Patient Access, you can view, book and cancel appointments from home, work or on the move - wherever you can connect to the internet.
To use Patient Access you will need a Practice ID number and Access ID number which is obtained from the practice, and, in conjunction with a password, will be unique to you.

You can download and print many of our administrative forms, sign them and return to the surgery by hand or via email. You will also have the opportunity to fill and sign them electronically using Adobe Acrobat Reader’s free Fill and Sign functionality
